Our commitment

Jarvis Medical Clinic protects the privacy of our patients. We collect, use and share personal health information only as permitted by Ontario's Personal Health Information Protection Act, 2004 (PHIPA). Dr. Olu Adeniyan is the health information custodian for the clinic.

What we collect and why

We collect the personal health information we need to provide your care, such as your contact details, health card number and medical history. We use it to treat you, to coordinate your care with other providers, and as otherwise permitted or required by law.

This website

This website does not ask for health information, and the site itself sets no tracking cookies. The booking calendar is provided by Google Calendar, and the map by Google Maps; their own privacy terms apply when you use them. Please do not include health details when you book.

The new patient registration button opens an online intake form hosted by a third-party provider. Information you enter there is handled under this policy.

Your rights

You may ask to see or get a copy of your health record, ask us to correct it, or ask us not to share it with certain people. Contact the clinic to make a request.
